NSCTNG 002	REQUEST FOR TRAINING AUTHORITY OFFICER (Rev 01/06) Use to request Officer/Instructor/Midshipman orders from NHQ for nationally organized training.
NSCTNG 003	LOCALLY ARRANGED TRAINING AUTHORITY CADET (Rev 04/07) Use to authorize NSCC/NLCC Cadets for up to five 5 days of locally arranged training or ten 10 days of training with the US Coast Guard.
NSCTNG 004	LOCALLY ARRANGED TRAINING AUTHORITY OFFICER (Rev 01/06) Use to authorize NSCC/NLCC Officers, Instructors, and Midshipmen for up to five 5 days of locally arranged training or ten 10 days of training with the US Coast Guard.
NSCTNG 005	NATIONAL TRAINING AUTHORITY CADET (Rev 04/07) Training authority granting permission for NSCC/NLCC Cadets to attend national organized Summer/Winter training evolutions. EXAMPLE ONLY- NHQ USE ONLY
NSCTNG 006	NATIONAL TRAINING AUTHORITY ESCORT OFFICER (Rev 02/04) Training authority granting permission for NSCC Officers, Instructors, and Midshipmen to attend national organized Summer/Winter training evolutions as Escort Officers. EXAMPLE ONLY- NHQ USE ONLY
NSCTNG 007	NATIONAL TRAINING AUTHORITY COTC (Rev 02/04) Training authority granting permission for NSCC Officers, Instructors, and Midshipmen to attend national organized Summer/Winter training evolutions as the Commanding Officer of the Training Contingent. EXAMPLE ONLY- NHQ USE ONLY
NSCTNG 008	NSCC/NLCC CADET TRAINING ROSTER (Rev 10/00) Use to provide COTCs with pertinent contact information and roster of personnel attending training.
NSCTNG 009	COTC TRAINING REPORT (Rev 02/02) Used by Commanding Officer Training Contingent COTC for status of training to NHQ. COTC USE ONLY
NSCTNG 010	TWO WEEK TRAINING AUDIT REPORT (Rev 03/06) Used by Commanding Officer Training Contingent COTC for financial reporting to NHQ. COTC USE ONLY
NSCTNG 011	TRAINING AUTHORITY TERMINATION REPORT (Rev 02/02) Used by Commanding Officer Training Contingent COTC to report to NHQ, Field Representatives, Regional Directors, and Unit Commanding Officers as to reasons why a Cadet was returned home from Two Week Training TWT. COTC USE ONLY
NSCTNG 012	UNIT TRAINING PREPARATION EVALUATION (Rev 12/00) Used by Commanding Officer Training Contingent COTC to report to NHQ, Field Representatives, Regional Directors, and Unit Commanding Officers as to the quality and conformity to NSCC Regulations of personnel records of Cadets and Officers attending Two Week Training TWT. COTC USE ONLY
NSCTNG 013	NSCC/NLCC TRAINING CHECK OFF LIST (Rev 01/06) Used by Unit Commanding Officer to ensure that NSCC/NLCC Cadets are prepared physically, medically, logistically, and administratively for participation in training programs
NSCTNG 014	ADVANCEMENT EXAMINATION (REQUEST) (Rev 01/00) Used by Commanding Officers to request Petty Officer Leadership Examinations from Regional Director/Headquarters Representative.
NSCTNG 015	ADVANCEMENT EXAMINATION (INVENTORY) (Rev 01/00) Used by Regional Directors/Headquarters Representatives to account for issued Military Leadership Examinations. FIELD REPRESENTATIVE USE ONLY NO LONGER USED (See Action Letter 04-06)
NSCTNG 016	ADVANCEMENT EXAMINATION (DESTRUCTION) (Rev 01/00) Used by Regional Directors/Headquarters Representatives to account for issued Military Leadership Examinations that have been destroyed. FIELD REPRESENTATIVE USE ONLY NO LONGER USED (See Action Letter 04-06)
NSCTNG 017	COAST GUARD STATEMENT OF UNDERSTANDING (Rev 03/01) Check off list for Commanding Officers, Cadets, and Parents to understand the requirements of participation in the Summer Training Program with the United States Coast Guard.
NSCTNG 018	SEAL PHYSICAL FITNESS REPORT (Rev 02/02) Physical screening form used to test applicants for NSCC SEAL Training.
NSCTNG 019	NSCC SCHOOL OF MUSIC PROFESSIONAL REFERRAL (Rev 02/02) Experience screening form used to provide referral/recommendation on Cadets applying for NSCC School of Music.
NSCTNG 020	NSCC/NLCC CADET EXERCISE CHART (Rev 02/02) Use track NSCC/NLCC Cadet exercise performance for semi-annual and AT participation screening.
NSCTNG 021	COTC REQUEST FOR FUNDING (Rev 04/07) Request for Federal Funding of Nationally schedules summer training programs. Additional instructions. COTC USE ONLY
NSCTNG 022	REQUEST FOR REIMBURSEMENT COAST GUARD (Rev 02/03) Request for reimbursement of training fees for training at approved US Coast Guard activities. Additional instructions.
NSCTNG 023	REQUEST FOR REIMBURSEMENT LOCAL (Rev 02/05) Request for reimbursement of training fees for training at pre-approved locally arranged training sites. Additional instructions.
NSCTNG 024	ESCORT OFFICER'S QUESTIONNAIRE (Rev 02/02) For use by NSCC Officers, Instructors, and Midshipmen interested in applying for Escort Officer billets.
NSCTNG 025	MEDICAL HISTORY SUPPLEMENTAL FOR TRAINING AUTHORIZATION, CONSENT AND RELEASE (Rev 03/08) For use by NSCC/NLCC Cadets who are currently are taking prescription or non-prescription medications. This form is only accepted by COTCs have the ability and staff to handle these medical issues.
NSCTNG 026	RECRUIT THREE GET A TRAINING FREE DEPOSIT CERTIFICATE (Rev 03/08) For use by Unit Commanding Officers, Regional Directors, and Commanding Officers of Training Contingents to award training deposit credits for Cadets who recruit three members into the NSCC/NLCC program. See Information Letter 07-08 for additional information.
